Verifying Benefits

• If for some reason the provider's office is not able to verify your benefits, you need to specify that you are insured with Golden Rule Insurance Company a subsidiary of UnitedHealthcare

• You are part of the UHC Choice Plus network, which is the largest network in the country.

​

• If the provider wants to verify benefits with your Insurance carrier, they need to call the UnitedHealthcare phone number: +1(800) 657-8205. This is the same number that is on the back of your Member ID Card.
